  • The Client Rights Officer at FrontLine Service can be reached at 216-623-6555 (please ask for The Client Rights Officer) during the hours of 9:00 a.m. to 5:00 p.m. Monday through Friday or you can send an email.  The Client Rights Officer will help you to file your complaint or grievance if you need help; will investigate your complaint or grievance; and, if you want, he/she will help you at the agency hearing about your complaint.

Mental Health Resources


  • Medicaid, call 216-987-7000 for location of welfare office.  If you are a family with children, you can call 216-987-7346 and complete the application by phone and mail.  Medicare Website.
  • Social Security Number/Card.  Social Security Administration.

Birth Certificates



  • Food Stamps, Cuyahoga County Employment and Family Services
  • Food Pantries, call 211 or 216-436-2000 for the phone number of the pantry that serves your area.
  • Street Card.  The Street Card is a publication of the Northeast Ohio Coalition for the Homeless. It is updated yearly and contains information that would assist a homeless person find access to services in Cuyahoga County.
